Patient Resources

Scheduling Appointments and Patient Forms

At Envision Eye & Laser, we understand and appreciate that your time is valuable! Dr. Bovelle and her staff will make every conceivable effort to see her patients at their scheduled appointment time. Please note that while our office may fall behind on occasion we will try to make your wait time as pleasant as possible. You may want to bring a book, newspaper, paperback, etc. In return we ask that you provide us with advanced notice if you may be running late for an appointment or if you need to cancel. Our patients are expected to cancel “at least 24 hours in advance” of their scheduled appointment in order to avoid being charged a $25.00 No-Show Fee.  Again, if you cancel an appointment within 24 hours of your appointment  you may be charged twenty five ($25.00) dollars. Please do everything possible to avoid this!

First Time Patients

First time patients should arrive early in order to allow for extra time to complete patient registration forms. Please allow two hours for your appointment because your eyes will be dilated. After dilation your eyes will be sensitive to light, so you should bring sunglasses with you and you may want to bring someone to drive you home.  Also, it will be hard to read after several hours. To save time, please print and fill out the following forms and bring them with you to your appointment:

  • Patient History Form
  • Registration Sheet
  • Practice Financial Policy
  • Refraction Policy

What to Bring to Your Appointment

In addition to these forms, please bring the following items to the appointment:

  • Your insurance card(s)
  • List of all prescription and over-the-counter medications that you are currently taking
  • Any required co-payment
  • Pertinent medical and surgical history information
